Senior Transitions Can Assist in Finding Answers
Senior Transitions provides:
· A meeting with our Licensed Social Worker in person or on the phone.
· Education about the aging process.
· Assessment of the individual’s needs.
· Information about resources available to assist the individual as needs change.
· Facilitating family meetings.
· Assistance with developing positive care giving approaches.
· Referrals to other professional resources that would benefit the individual and his or her family.
· Geriatric Case Management can arrange and monitor services to ensure that older adults enjoy the best possible quality of life.
· Act as a liaison to families.
· Provide crisis intervention.
· Advocate for the older adult.

All but one service through Senior Transitions is offered without charge. Donations to the Senior Center are accepted. There is a fee scale for Geriatric Case Management. Ask our Senior Transitions Coordinator for details. The Senior Transitions Program was launched with a program grant through the Community Foundation of Wabash County.
